Evaluation and Referrals

Wellstone Regional Hospital offers no cost health care assessments 24 hours a day, 7 days a week. The Evaluation and Referral department is staffed by both registered nurses and masters-level social workers that are always available to complete assessments. A patient can either call and make an appointment or simply walk in for an assessment. Once the patient comes to Wellstone for their appointment, they will meet with a clinician in a private area to complete the health care assessment. Once the assessor gathers all information from the patient, the assessor will call the psychiatrist and review the assessment. The psychiatrist then makes the level of care determination for either inpatient or outpatient services.
