Evaluation & Referrals

The first step towards receiving help and feeling better is to obtain an assessment in the Wellstone Evaluation and Referral department. No appointment is necessary, and there is no charge for this service. The department is open 24 hours, 7 days per week.

The assessment takes about an hour, in which a trained staff member of the clinical team will meet with you in an office and confidentially ask you a series of questions. We take the time to listen to your concerns, symptoms, thoughts, and feelings.

After the assessment is completed, the clinical team member consults with our on-call psychiatrist and, together, they will identify the most appropriate level of care for you. Treatment options include Outpatient, Intensive Outpatient, Partial Hospitalization, and Inpatient Care. For Outpatient referrals, we are able to identify up to three outpatient treatment providers that can meet your needs.

At the conclusion of the assessment, recommendations are made and you will be on your way to Quality Care and Peace of Mind.
